At Ironwood OB/GYN, Alice M. Halscheid, practice administrator, lead and managed a successful EMR system implementation. So what did it take to get there?</p><p>&#13;</p><p>Preparation. Going to an EMR solution should not be a rash decision. After 3 years of research, watching the main stream EMR service providers, closely examining cost and extent of training, as well as making inquiries of other OB/GYN offices nationwide, Ironwood justified an EMR solution investment. Realizing the initial investment should be recovered via reimbursement through a more efficient means of billing, which helped to support the foundation of the investment.</p><p>&#13;</p><p>Team Leaders and Communications. The physicians (providers) and practice administrator had many, many conversations about the transition from paper to paperless. The paradigm shift would present a steep learning curve, but the providers knew that in order to improve patient flow, treatment and care, they needed to make the investment. That commitment set the communications tone in the office.</p><p>&#13;</p><p>•	EMR systems were a focal point of discussion at staff retreats, monthly meetings, and so on. Staff and providers were encouraged to communicate via email. Providers were also encouraged to get on their computers every day to stay connected with the “EMR movement” by reading their emails.</p><p>&#13;</p><p>•	The physicians decided among themselves who would be the &#8220;champion physician(s)&#8221; for visiting other OB/GYN offices to see and ask how they implemented and used their software.</p><p>&#13;</p><p>•	Certain service providers and physician communications improved, based on the flow of the visit, or Treatment Plans because each physician had their own way of seeing patients &#8212; breast examinations, paps, blood testing, questions and answers based on the patient&#8217;s history.  The treatment plans drive the entire EMR program, allowing for total documentation.</p><p>&#13;</p><p>Assessment &amp; Analysis. Ironwood concentrated on those software programs that seemed to be the most popular. It was time consuming, but very necessary. They needed to make sure the assessment was inclusive, not exclusionary. Office visits to the medical practices that used those programs followed.</p><p>&#13;</p><p>•	Seeing those programs that provide for the ACOG format for seeing OB patients and capturing the flow needed to accommodate seeing a patient for up to 8 months of pregnancy was a key requirement. From there, that service provider was again to the Ironwood office, which was a 3rd visit.</p><p>&#13;</p><p>•	Program selection was narrowed to three main systems that staff and providers believed to be quite functional for a OB/GYN specialty.  While many EMR solutions are feature rich and functional, many were not a good fit for the OB/GYN patient flow.</p><p>&#13;</p><p>•	Due diligence included financial stability, system dependability, cost factors, training presentations, and validity of statements made by the company.</p><p>&#13;</p><p>•	It is known and expected that there is no guarantee that any one company will really be better than another. However, Ironwood needed to rely on its own sense and sensibility when making such a large purchase.</p><p>&#13;</p><p>Implementation. The conversion effort to digital records was relatively easy. Staff and providers had to change some plans as Ironwood moved forward, realizing that the initial plan was not as effective as it could have been, but overall the transition has been relatively easy.</p><p>&#13;</p><p>The practice has begun to scan entire charts into the system, which will enable the physicians to review a patient&#8217;s chart from the convenience of their home. They no longer spend hours looking for lost charts through the office. Believe it or not, medical records positions are becoming obsolete, giving way to a leaner more efficient staff.</p><p>&#13;</p><p>Faster and Sooner<br
/>&#13;</p><p>Ironwood would have gone to an EMR system much sooner! It is amazing how efficient it is. There are still are some behavioral stumbling blocks however, in that, it takes the physicians sometimes a little longer to complete a patient’s chart because the software asks so many questions; it does not permit the &#8220;skipping&#8221; over some of the important information. No question about what it does – the system encourages efficient patient care.</p><div

Computers, monitors, cell phones, DVD players, copy machines, TV sets… you get the picture.</p><p>&#13;</p><p>Most, if not all of this electronic waste, contains hazardous chemicals like mercury, lead and chromium that end up leaching into our ground water from landfills or being emitted into the air from incinerators causing a GROWING environmental and health crisis. This problem is fueled by the fast paced growth of new technology that renders electronic products obsolete long before their normal lifecycle ends and the fact that most people do not know how to properly dispose of these types of products. In fact, most don’t know that these products can be recycled and reused.</p><p>&#13;</p><p>But discarded electronics can be properly disposed of or recycled and it is easier than most people think.  Tons of companies have programs that allow you to donate usable electronics to churches and schools, and other companies will recycle usable components, raw materials and materials like plastics, glass and aluminum.  Many cities also have e-waste pick up as part of their hazardous waste pick-up programs.</p><p>&#13;</p><p>You can even take advantage of tax breaks when you donate your unwanted electronics.  The 21st Century Classrooms Act for Private Technology Investment allows large companies to donate used equipment to public and private schools for tax breaks and donations to nonprofits can be written off.  You will need to check with each nonprofit organization regarding its particular documentation for your tax returns. </p><p>&#13;</p><p>The environmental benefit to donating or recycling used electronics goes far beyond just keeping harmful chemicals out of our landfills; it also helps conserve our precious natural resources.  It takes approximately 530 pounds of fossil fuel, 50 pounds of chemicals and 416 gallons of water to produce one desktop computer.  Recycling electronic components can help preserve these resources and can also help use far less energy than the production of new components.  Mining of aluminum, for example, uses 20 times the amount of energy it takes to recycle the same amount of aluminum from electronic components.  Last year alone, electronics recyclers recovered over 100 million pounds of materials like aluminum, steel, glass and plastic which was reused.</p><p>&#13;</p><p>Now that we know the benefits of e-cycling let’s, figure out how to find an e-cycler in your area.  You can start with the <a
rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.eiae.org/">Electronic Industry Alliance</a> . This site has an easy to use map that allows you to search for e-cycling by state and offers you several other helpful links to guide you through the process.  You can also check with your city government to see if it offers e-cycling as part of its hazardous waste programs or with <a
rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.goodwill.org/">Goodwill industries</a> . There are several additional organizations you can check that offer training programs that teach students and individuals how to refurbish used electronics which are then donated to local schools:</p><p>&#13;</p><p>Students Recycling Used Technology (StRUT)</p><p>&#13;</p><p>Learning and Information Networking for Community via Technology (LINCT)</p><p>&#13;</p><p>Reuse Development Organization (ReDO)</p><p>&#13;</p><p>Even Sam’s Club has an interesting program that runs in cooperation with N.E.W. Customer Service Companies, Inc. Members of the organization who want to recycle computers, LCD monitors, printers, camcorders, digital cameras and MP3 players, can log onto the program’s website and print a FREE shipping label to have the products mailed to N.E.W. Those members will receive a Sam’s Gift Card for the value of the donated item.  All donated items are either refurbished or disassembled and the parts are used to rebuild electronics or recycled into raw material and reused.</p><p>&#13;</p><p> The final pieces to this recycle cycle are personal data security and rethinking your purchasing habits.  You need to protect all your personal data contained on computer hard drives and cell phones.  Please make sure to remove ALL stored phone numbers and call logs from your cell phone and you may even want to remove the SIM card to insure protection.  Cleaning up your computer may prove a little more difficult but is equally important.  If you are unsure of how to do this you can find FREE hard-drive erasers through your favorite search engine. Type in “free hard-drive eraser cleaners.”</p><p>&#13;</p><p>Although there are no laws governing the use of hazardous materials in electronics equipment you can help to encourage manufacturers to move in a “greener” direction by considering the following when buying your next piece of equipment:</p><p>&#13;<br

/>Are the products designed for easy upgrades and disassembly?<br
/>Does the manufacturer offer a lease or “take back” program?<br

/>Does the manufacturer use minimal or recyclable packaging?</p><p>The age of electronics is upon us and with the constant introduction of new technology, system upgrades and fancy new gadgets coming at us daily we need to be more conscious of what we do with our e-waste. The filter contains an atomizer and a cartridge of nicotine, suspended in liquid. When you take a drag, the atomizer converts this into a vapor that looks like smoke, but isn’t.</p><p>When my e-fag works, it is effective. There is even a delightful orange LED that lights up when I take a drag. But half the time it doesn’t work properly. The cartridges, which are supposed to be equivalent to 12 fags, run out at alarming speed. Am I sucking too hard? Am I taking in far more nicotine (technically a poison, and a vasoconstrictor to boot) than I did as a regular smoker? It is impossible to say.</p><p>Most e-cigarette vendors are careful not to market them as a quitting aid, an attitude echoed by the World Health Organization, which has aired concerns about them not having been subject to regulation and peer-reviewed clinical trials. Psychokenisis connotes the ability to move objects with your mind. Pareidolia means that you mistakenly perceive images and sounds as being recognizable. A man in the moon, a face in ripples of glass windows, or hearing messages on records played in reverse are keen examples of pareidolia.</p><p>&#13;</p><p>Most EVP sounds are in short, abrupt segments, usually the length of a word or phrase; sentences are uncommon, but not unheard of. The segments are frequently heard in the language of the listener.</p><p>&#13;</p><p>A psychologist, Konstantin Raudive, conducted over 100,000 recordings under different conditions. His research amassed some conclusions about elements that all EVP sounds share. They used an altered rhythm compared to customary speech, were short in duration and resembled telegram-like speech, did not follow grammatical guidelines and rules, and several languages were heard over the space of a single recording.</p><p>&#13;</p><p>Possible explanations, paranormal and non-paranormal, have been proposed and scrutinized by researchers and laymen.</p><p>&#13;</p><p>A paranormal explanation, for example, is the idea that bodiless, ethereal spirits, in the absence of their own vocal cords, imprint their messages on recording media through some elusive method. Another is that extraterrestrials communicate, intentionally or accidentally, through some blip in space-time. The third most common idea is psychokenisis in which the subject is said to possess influence over matter with his mind. This term is popular in parapsychology.</p><p>&#13;</p><p>Non-paranormal and scientific explanations include interference,  pareidolia, capture errors, processing artifacts, and hoaxes. Interference is common when EVP phenomenon is recorded on devices that contain RLC circuitry. The sounds are, evidently, voices and sounds from broadcast radio sources. Capture errors are anomalies created by the overamplification of a signal at the time when it was initially recorded. A plethora of odd noises can result from it. A processing artifact is a sound that results from attempts to boost the clarity of an existing signal. I.e., frequency isolation, re-sampling, and noise reduction and enhancement can all conspire to create a sound that is artificially unique in comparison to the original.</p><p>&#13;</p><p>Important researchers of the past and present are notable in the course of your further studies in electronic voice phenomenon. Some names to remember are Attila von Szalay, Raymond Bayless, Alexander MacRae, Judith Chisholm, Konstantin Raudive, Friedrich Jurgenson, Hans Bender, William O&#8217;Neil, and Sarah Estep. Many of these researchers made strides in exploring and popularizing EVP, but they don&#8217;t represent a good sample of current researchers. This is because there are very few researchers today. There are scant articles in peer-reviewed journals, but EVP continues to be ignored by scientists at large. Experiments have produced mixed results. EDI also is used within individual organizations to transfer data between different divisions or departments, including finance, purchasing and shipping. When the focus of EDI centers on payments, especially between banks and companies, the term financial EDI (FEDI) is sometimes used. Along with digital currency, electronic catalogs, intranets and extranets, EDI is a major cornerstone of e-commerce overall.</p><p>Two characteristics set EDI apart from other ways of exchanging information. First, EDI only involves business-to-business transactions; individual consumers do not directly use EDI to purchase goods or services. Secondly, EDI involves transactions between computers or databases, not individuals. Therefore, individuals sending e-mail messages or sharing files over a network does not constitute EDI.</p><p>While the concept of e-commerce did not receive widespread attention until the 1990s, large companies have been using EDI since the 1960s. The railroad industry was among the first to adopt EDI, followed by other players in the transportation industry. By the early 1980s, EDI was being used by companies in many different industry sectors. In the beginning, companies using EDI transferred information to one another on magnetic tape via mail or courier, which had many drawbacks including long lead times and the potential for a tape to be damaged in transit. During the 1980s, telecommunications emerged as the preferred vehicle for transferring information via EDI.</p><p>By the new millennium, EDI was used widely in many industries including manufacturing, finance, and retail. Some large retailers, among them Sears and Target, required suppliers to use EDI in order to engage in business transactions with them. Additionally, the Federal Acquisition Streamlining Act of 1994 (FASA) required all agencies within the United States government to use EDI.</p><p><strong>Communication Methods.</strong></p><p><p>After identifying trading partners, entering into TPAs with them and purchasing the necessary hardware and software, a means of communication must be chosen. EDI can occur point-to-point, where organizations communicate directly with one another over a private network; via the Internet (also known as open EDI); and most commonly, via value-added networks (VANs) provided by third-party value-added-network services.</p><p>VANs are networks dedicated exclusively to EDI. Not only do they function like telephone lines by allowing for the transfer of information, they also contain storage areas, similar to e-mail boxes, where data sent from one party can be held until it is scheduled to be delivered to the receiver. VANs are able to provide translation services to small organizations that find it too cost prohibitive to do in-house with their own software. Companies may need to join more than one VAN because their partners belong to more than one. However, by the early 2000s most VANS were able to communicate with one another.</p><p>In addition to translation, VANs offer a wide variety of other services including data backup, report generation, technical support, training, and the issuance of warnings if data is not properly transmitted between parties. Depending on need, all of the services offered by a VAN may not be required by a particular company. VANs vary in the way they charge companies. Some charge high implementation or setup fees followed by low monthly usage fees, or vice versa. Charges often are made based on the number of documents or characters involved in a given transmission. For example, one EDI provider charged its clients a monthly mailbox fee of $17.50, followed by a charge of 30 cents per 1,024 characters (per kilo character or k/char) transmitted. Additionally, charges can vary depending on participants&#8217; phone companies and the time of day when transactions are made. It can be less expensive for companies to make transactions during off-peak or evening hours.</p><p>In the early 2000s, although many companies still relied on VANs, the Internet was playing a larger role in EDI. It is possible for companies to translate EDI files and send them to another company&#8217;s computer system over the Internet, via e-mail or file transfer protocol (FTP). Because it is an open network and access is not terribly expensive, using the Internet for EDI can be more cost effective for companies with limited means. It has the potential to provide them with access to large companies who continue to rely on large, traditional EDI systems. The low cost associated with open EDI also means that more companies are likely to participate. This is important because the level of value for participants often increases along with their number. However, this also presents a dilemma for large companies who have invested a considerable sum in traditional EDI systems. Furthermore, Internet service providers (ISPs) usually do not offer the kinds of EDI-specific services provided by VANs.</p><p>While the automotive and retail industries have experimented with open EDI for some time, the efforts didn&#8217;t result in widespread adoption by small suppliers, usually due to cumbersome requirements like the installation of on-site software. Incorporating EDI into e-marketplaces was an approach that held more potential. In March 2000, an e-marketplace called the WorldWide Retail Exchange (WWRE) was established. It allowed suppliers and retails in various industry sectors—including retail, general merchandise, food and drugstores—to conduct transactions over the World Wide Web. After one year of operation, the WWRE had 53 retailer members with combined annual turnover of $722 billion. Leading retailers, among them Kmart, Rite Aid, Best Buy, and Target, planned to offer a Web-to-EDI translation service on WWRE so it would be easier for smaller suppliers to do business with them. In this arrangement, the retailers send purchase orders to a data center where they are translated to a language that can be read with a Web browser like Internet Explorer or Netscape Navigator. Suppliers are then notified about the PO and allowed to respond. This is a break from true EDI, since orders are handled manually by suppliers.</p><p><p>In addition to the Internet, intranets (private internets) and extranets (links between intranets and the Internet) also showed potential for EDI. According to The International Handbook of Electronic Commerce, &#8220;The Extranet makes it possible to connect several organizations behind virtual firewalls. For example, suppliers, distributors, contractors, customers, and trusted others outside the organization can benefit from establishing an Extranet. The Internet is used to provide access to the public; the Intranet serves the internal business; Extranets provide a critical link between these two extremes. Extranets are where the majority of business activity occurs. They enable commerce through the Web at a very low cost and allow companies to maintain one-to-one relationships with their customers, members staff and others.&#8221;</p><p><strong>Communication Standards.</strong></p><p><p>As previously mentioned, when companies use EDI to exchange information, translation software is an important part of the process. During EDI, information is usually translated to and from one of several different standard languages, including ANSI X12 and EDIFACT. These languages are more flexible than custom standards developed by individual companies for their specific use.</p><p>Because of its reliability and flexibility, ANSI X12 was the most widely used North American standard in the early 2000s. Also called ASC X12, ANSI X12 was developed by the American National Standards Institute (ANSI), which administrates and coordinates voluntary industry standardization within the United States. In addition to its prevelance in North America, this standard also was used in Australia and New Zealand.</p><p>Created in 1987 with the cooperation of the United Nations, Electronic Data Interchange For Administration Commerce and Transport (EDIFACT) standards combine the best aspects of ANSI X12 and a standard known as United Nations Guidelines for Trade Data Interchange (UNTDI). Because it is so universal, EDIFACT is suited for use in international EDI. Although EDIFACT was becoming increasingly popular in the early 2000s, it lacked the comprehensiveness of ANSI X12.</p><p>In addition to ANSI X12 and EDIFACT, other EDI standards also exist, including Global EDI Guidelines for Retail (GEDI), used within North America for international trade; the grocery industry&#8217;s Uniform Communication Standard (UCS); Voluntary Inter-Industry Commerce Standards (VISC), used by retailers of general merchandise; Warehouse Information Network Standard (WINS), used by public ware-houses; TRADACOMS, created by the Article Numbering Association and used by retailers in the United Kingdom; and NACHA, developed by the National Automated Clearing House Association and used for transactions in the banking industry. For companies using open EDI, a language called extensible markup language (XML), similar in some respects to hypertext markup language (HTML), allows users to share information in a universal, standard fashion without making the kinds of special arrangements EDI often requires and regardless of the software program in which it was originally created.</p><p><strong>How Edi Works</strong></p><p><p>During EDI, information is sent from one participant&#8217;s computer system and translated to a standard format with special translation software. It is then transmitted to another participant, translated back from the standard format into a format used by the receiver and entered into the receiver&#8217;s computer system. Thus, EDI allows participants to transfer information between their respective computer systems, even if the systems utilize different, incompatible platforms.</p><p>Before using EDI, companies usually enter into specific agreements with their trading partners (called trading partner agreements or TPAs). These contracts often spell out the kinds of information they will exchange and how they will exchange it. Because entering into and terminating TPAs is expensive and time consuming, traditional EDI isn&#8217;t always ideal for companies who change suppliers often, or for companies who frequently enter into temporary relationships with suppliers or other companies.</p><p><p><strong>Why EDI?</strong></p><p><p>· EDI comes into its own when repetitive manual tasks are required to support a business relationship; Electronic data interchange simply eradicates them by automating the process and removing the paperwork element.</p><p><p>· It increases accuracy by eliminating the re-keying of data and the quality of data is enhanced by agreeing product codes, prices and location codes in advance.</p><p><p>· Electronic data interchange also helps cement customer/supplier partnerships by reducing the supply chain costs associated with manual processing.</p><p><p>· EDI enabled suppliers are cheaper and easier to deal with.</p><p><strong>Advantages of EDI</strong></p><p><p>Companies use EDI to exchange information for a variety of different reasons, mainly increased efficiency and cost savings. For example, EDI allows business transactions to occur in less time and with fewer errors than do traditional, paper-based means. It reduces the amount of inventory companies must invest in by closely tying manufacturing to actual demand, allowing for just-in-time delivery. By doing away with paper forms, EDI also reduces postage costs and the expenses and space considerations involved in paper-based record storage. Some companies have seen dramatic improvements in their business processes, such as the shortening of delivery times from days to hours. However, other EDI users have continued to experience snags. In Planet IT, Procter &amp; Gamble, a leading packaged goods manufacturer, reported that it found errors in more than 30 percent of its electronic orders, although these were mainly due to human mistakes.</p><p>Although many companies don&#8217;t view EDI as a strategic weapon, it certainly can be used as one. Having the capability to engage in EDI is a marketing tool, because it makes suppliers attractive to retailers and other companies who buy goods and services. In a situation where several suppliers offer similar products, being EDI-enabled can be an important differential. In 2000 alone, business-to-business sales were estimated to be $3.3 trillion, with forecasts predicting an increase to $5.2 trillion by 2004, according to Corporate EFT Report.
